Position Summary
The Community Outreach Specialist conducts community-based engagement, phone outreach, and in-home visits to support community members. This role performs point-of-care blood pressure verification using electronic cuffs, addresses medication adherence through in-person outreach and warm transfers to adherence vendors, and provides limited courier services for medication pickup and delivery. The ideal candidate demonstrates cultural humility, strong communication skills, clinical comfort with basic vitals collection, and a commitment to serving underserved and vulnerable communities.
Key Responsibilities
- Conduct phone outreach to assigned members to schedule home visits and complete warm transfers to the medication adherence vendor.
- Conduct in-person adherence outreach, including visits to members identified as non-adherent or at risk of non-adherence.
- Measure and document blood pressure using organization-provided electronic cuffs to verify compliant readings for program requirements.
- Provide courier services for the pickup and delivery of medication to assigned members, adhering to chain-of-custody and confidentiality procedures; a travel stipend is provided.
- Accurately document encounters and complete daily data entry, case notes, and reports on schedule.
- Coordinate with internal teams and external partners to facilitate referrals and ensure continuity of care.
- Meet individual and team performance goals.
- Participate in daily, weekly, and monthly team meetings and required training.
- Deliver culturally competent, trauma-informed interactions that respect clients’ backgrounds and needs.
Qualifications
- Associate degree or equivalent professional experience.
- Minimum two years of community outreach, casework, or phone-based engagement preferred.
- Comfort and experience with basic vital signs collection preferred; training on electronic BP cuffs will be provided.
- Experience or demonstrated competency in community outreach is preferred.
- Reliable transportation, a valid driver's license, and proof of insurance.
- Availability for a flexible schedule that may include daytime and weekends.
- Proficient with basic computer applications and willing to learn new technologies and databases.
- Strong verbal and written communication and interpersonal skills.
- Excellent time management and ability to work independently.
- Bilingual skills in any language are a plus.
- Ability to pass pre-employment background check, drug screen, and random drug testing as required.
Working Conditions and Physical Requirements
- Frequent travel within the assigned geographic area and occasional longer trips.
- Regular in-home visits that may require navigating stairs, carrying light supplies and equipment, and working in varied environments.
- Responsibilities include lifting and transporting medication packages of moderate weight as part of courier duties.
- Hybrid mix of fieldwork and call center duties with regular computer use and phone communication.
- Use of electronic blood pressure cuffs and adherence vendor telephony during visits.
