Annual Salary:$18 - $30/hr + travel stipend (commensurate withexperience)

We're looking for a passionateSocial Care Partnerto join our team inIndianapolis, IN. In thiscontract (potential for permanent role) you'll wear two hats: serving as a local champion for the Indianapolis community andbecoming a role model and trusted subject-matterexpert to Associate-level outreach team members.

Responsibilities:

  • Indiana Community Champion:
    • Build relationships and trust within the Indianapoliscommunities, especially with vulnerable populations and with local communityorganizations
    • Design and implement impactful outreach strategies that address community needs
    • Lead in-person outreach efforts at community events, connecting residents with resources
    • Lead and conduct home visits, providing personalized support and assistance as needed
    • Serve as a liaison between CareAdvisors and Indianapolisresidents, ensuring their voices are heard

  • Trusted Role Model & Support:
    • Coach, mentor, andguide Associate CommunityOutreachSpecialists
    • Model empathetic andcompassionateleadership atall times
    • Leverage your knowledge of local community organizations and resources to helpAssociate members increaseknowledge and improve outreach skills
    • Assistleadership with onboarding andtraining new team members
    • Stay up to date onlocal resources andinformation in order to knowledge-share
    • Model and advocatefor meticulous data-tracking & documentation in our systems
    • Collaborate with the Social Care Manager on outreach efforts to expand resident knowledge of resources
Qualifications:

  • When needed, must be OK with traveling 1-2 hours to complete a Visit (travel time is paid)
  • Bachelor's Degree in Human Services or related field (or commensurate experience)
  • 4+ years of experience in a hospital, social services setting, or community outreach
  • Strong understanding of case management, Medicaid, and government benefits (or 4+ years in behavioral health)
  • Bilingual a plus (Spanish preferred)
  • Excellent interpersonal and social skills to connect with diverse populations
  • Strong written and verbal communication to effectively advocate formembers
  • Valid driver's license, proof of insurance, and access to a vehicle
  • Proficiency in computer applications and willingness to learn new technologies
