Location: Travel within INDIANA

Reports To: Social Care Manager

Annual Salary: $23 - $30/hr (commensurate with experience)

CareAdvisors is a healthcare technology company on a mission to dismantle barriers to care for vulnerable and underserved communities. We're looking for a passionate Community Outreach Partner to join our team in Indianapolis, IN. In this contract-to-permanent role, you'll wear two hats: becoming a trusted subject-matter expert to Associate-level team members and serving as a local champion for the Indianapolis community.

Responsibilities:

  • Trusted Social Care Specialist:
    • Manage complex cases through communication with patients, caregivers, and social workers
    • Leverage deep knowledge of Medicaid and other government benefits to guide clients through applications
    • Offer information and referrals to community services, ensuring smooth transitions
    • Document cases meticulously in EHR systems
    • Collaborate with the Social Care Manager on outreach efforts to expand resident knowledge of resources
  • Indiana Community Champion:
    • Build relationships and trust within the Indianapolis communities, especially with vulnerable populations and with local community organizations
    • Design and implement impactful outreach strategies that address community needs
    • Lead in-person outreach efforts at community events, connecting residents with resources
    • Lead and conduct home visits, providing personalized support and assistance as needed
    • Serve as a liaison between CareAdvisors and Indianapolis residents, ensuring their voices are heard
Qualifications:

  • Bachelor's Degree in Human Services or related field (or commensurate experience)
  • 4+ years of experience in a hospital, social services setting, or community outreach
  • Strong understanding of case management, Medicaid, and government benefits (or 4+ years in behavioral health)
  • Bilingual a plus (Spanish preferred)
  • Excellent interpersonal and social skills to connect with diverse populations
  • Strong written and verbal communication to effectively advocate for members
  • Valid driver's license, proof of insurance, and access to a vehicle
  • Proficiency in computer applications and willingness to learn new technologies

Compensation & Benefits:

  • Salary: commensurate with experience
  • Flexible schedule - we can work with you around your other commitments
  • Part-time, Contract to start; ideally we are looking for a long-term Indiana hire after a 3 month contract
