Location: Baltimore, MD (travel within Baltimore area)

Reports To: Social Care Manager

Annual Salary: Hourly + Travel Stipend

About the Role: CareAdvisors is seeking a Community Outreach Partner to spearhead our community engagement initiatives in Baltimore. We are looking for a local champion deeply connected to the needs of their community and passionate about advancing health equity and social justice. In this role, you will collaborate closely with us to design and implement impactful outreach strategies with the community's needs at the forefront.

Core Responsibilities

  • Serve as a liaison between CareAdvisors and local communities, ensuring that their voices are heard and their needs are addressed
  • Organize and conduct home visits within designated communities, providing personalized support and assistance as needed
  • Lead in-person outreach efforts at community events, disseminating information on available resources and facilitating enrollments
  • Provide timely assistance and resources to community members, responding sensitively to their cultural and socio-economic backgrounds
  • Offer post-event support, including follow-ups with participants to ensure continuity of care and support
  • Complete necessary documentation and data entry accurately and promptly
  • Meet goals set by leadership and actively participate in team meetings and discussions
  • Undertake other tasks as assigned by leadership

Core Requirements

  • Deep commitment to community engagement and advancing health equity
  • Associate’s degree or equivalent professional experience
  • Minimum of 2 years of experience in community outreach or related field
  • Availability for a flexible work schedule, which may include weekends
  • Access to a vehicle, valid driver’s license, and proof of insurance
  • Basic computer skills and willingness to learn new technologies
  • Strong time management and organizational skills
  • Excellent verbal and written communication skills
  • Bilingual proficiency preferred
  • Ability to pass pre-employment background checks and drug screenings
  • Ability to work independently while effectively managing time
